This form is designed to help me get to know you, your hopes, preferences, and any support you might need throughout pregnancy, birth, and postpartum before we meet in-person. Please take your time answering; all information shared is kept confidential and used only to provide you with personalized, compassionate care.

There are no “right” answers. Just your story, your voice, your needs.

If you have any questions or prefer to go over these topics in person, feel free to skip anything you'd rather discuss during our prenatal visit.
